Occlusion of Arteriovenous Malformations of the Spinal Cord: Different Methods

Endovascular treatment of AVM of the spinal cord at the Burdenko Research Institute of Neurosurgery was carried out during more than 20 years. A total number of cases was 237. AVM of the spinal cord were located at different levels. Applied endovascular methods included occlusion of aneurysmís afferent vessels by a balloon according to Serbinenko F.A., thrombosing with polyvinylacetate (PVA) emboli, thrombosing with histoacryl. A total number of occluded vessels, participating in AVM blood supply at different levels, was 475. This work resulted in determining indications for use of this or that method of endovascular treatment of AVM under discussion.

There were 51 cases with AVM of the cervical spinal cord, which were occluded with a balloon (6), histoacryl (10) and PVA emboli (35). AVM, located at thoracic and lumbar levels, were revealed in 186 cases. Embolization was made with applying PVA emboli (134), a combination of detachable balloons and PVA emboli (13), histoacryl (39).

The results of endovascular treatment were as follows: complete restoration of neurological functions - 76 cases, improvement - 131 cases, no changes - 20 cases, aggravation of a state - 6 cases.

Today applied methods of endovascular occlusion are methods of choice in treatment of cases with AVM of the spinal cord, located at different levels.
