Head of the Department - Doctor of Medical Science, Prof. Panuntsev Vladimir Sogomonovich

The Department was organized in 1970 on the initiative of Prof. Ugriumov V.M. It was headed by Candidate of Medical Science Kesaev S.A. (1970-1985), Prof. Zubkov Yu.N. (1986-1997). Today its head is Prof. Panuntsev V.S.

The Department has 40 beds. Its staff consists of physicians of the first and highest qualification category, 2 Doctors of Medical Science and 8 Candidates of Medical Science, attentive and highly qualified nurses. The employees render diagnostic, therapeutic and surgical care to patients with different types of vascular pathology of the brain and spinal cord. Interventions are performed with application of new endovascular (intravascular) and microsurgical methods. All neurosurgeons of the Department have attained perfection in total and selective angiography of intra- and extracranial vessels with use of modern contrast substances. The Department was one of the first medical establishments which developed intravascular methods of treatment of various pathology of the brain. Today the following intravascular operations are performed:

- exclusion of cerebral aneurysms of different size and localization, their occlusion with balloons and coils;
- embolization of arteriovenous malformations (AVM) of the brain and spinal cord with the help of different materials;
- exclusion of carotid-cavernous and other arteriosinusal anastomoses of various etiology;
- angioplasty of extra- and intracranial arteries.

Operations for aneurysms, AVM, stenosing processes of intracranial and major cerebral arteries of various etiology are performed with application of microsurgical technique and monitoring. A combination of direct and intravascular interventions makes it possible to treat the most complicated types of vascular pathology. The system of diagnosis and complex treatment of stenosing and occluding lesions of cerebral vessels, AVM, a hemorrhagic period of intracranial aneurysms and other types of pathology of the brain and spinal cord is being developed and introduced.

New methods and principles of treatment of vascular diseases developed in the Department include:

- vasodilation in spasm accompanying aneurysm rupture;
- the scheme of prevention of secondary hemorrhages from aneurysms;
- the system of treatment of patients in an acute period of aneurysm rupture, which allows to reduce mortality and disability in this group;
- treatment of gigantic aneurysms with combination of direct and intravascular interventions.

Combined methods (intravascular, radiosurgical and open) used for surgical treatment of AVM of different size and localization allow to widen its capabilities in this category of patients. The system of complex treatment of stenosing and occluding lesions of cerebral vessels of various etiology includes both conservative and surgical methods. Surgical treatment consists in direct interventions on carotid and vertebral arteries, sympathetic cervical ganglia; intravascular angioplasty of major cerebral arteries with dilating balloon-catheters is used. Peculiarities of a clinical picture and morphology of cavernous angiomas is being studied; optimum ways of their treatment are being developed. Methods of surgical treatment of patients with pathologic arteriovenous shunting are applied depending on a type of a shunt.

The Department of Cerebrovascular Surgery takes an active part in advisory and educational activity. Doctors from many regions of the country specialize in neurosurgery in this Department.
